Third Party Terms

Practice Fusion, Inc. (“Practice Fusion”) facilitates certain Services to users who have entered into an Healthcare Provider User Agreement (“User Agreement”) with Practice Fusion through services provided by Envoy LLC, an Emdeon Business Services company (“Emdeon”). As a condition of your accessing Emdeon’s services, Emdeon requires that you agree to be bound by these additional terms (“EmdeonTerms”). These Emdeon Terms are cumulative with, and do not in any way limit, your obligations under your User Agreement; and, by accessing Emdeon’s services, you are thereby agreeing to these Emdeon Terms. Any terms or phrases not otherwise defined in these Emdeon Terms shall have the meaning given to them in your User Agreement.

With respect to Medicaid eligibility transactions:

1. You hereby agree that your access to eligibility information shall be restricted to the sole purpose of verification of Medicaid eligibility where the recipient has requested Medicaid payment for medical services.
2. You hereby agree that your verification of eligibility under the system is not a guarantee of payment and the records as to the recipient’s eligibility status shall be final authority.
3. You hereby agree to indemnify and hold harmless each state, its agents and employees, from any and all claims by you or any of the recipients who are aggrieved by your actions.

You hereby agree to abide by the federal and state regulations regarding confidentiality of information.
