Capture patient risk score with a few simple clicks
Practice Fusion’s new Patient risk score feature can assist providers with risk stratification and classification, which helps make it easier to identify patients who may need additional care management. You can now document patient risk scores for Hierarchical Condition Category (HCC), Adjusted Clinical Groups (ACG), Elder Risk Assessment (ERA), Chronic Comorbidity Count (CCC), and Minnesota Tiering (MN), or use the Other field to record practice-specific risk scores or any other risk score.
