Determine your Eligibility
Start by determining if you qualify for EHR incentives through either the Medicare or Medicaid EHR incentive program. Answer a few brief questions using the Centers for Medicare and Medicaid Services (CMS) Eligibility Widget below. Start by clicking "Next."
Depending on your Medicare or Medicaid program eligibility, you may need to click the Switch to Medicaid link above.
Register for the CMS EHR Incentive Program
If you are interested in EHR incentives, you first need to register for the Centers for Medicare and Medicaid Services EHR incentive program online. You only need to register once during your participation in the program.
When you register, have the following information handy:
- National Provider Identifier (NPI)
- National Plan and Provider Enumeration System (NPPES) User ID and Password. This is the same information that is used to log-in to PECOS.
- Practice Fusion's EHR Certification Number: 30000004H0RCEA0 (note all "0" are zeros)

Sign up for Practice Fusion e-Prescribing
CMS requires you to transmit more than 40% of prescriptions electronically in order to qualify for EHR incentives. If you haven't yet signed up for Practice Fusion's free e-prescribing module, follow the link above to download the required form and initiate the enrollment process.
You will need the following information to sign up for e-prescribing:
- Practice Fusion Practice ID and Username
- Provider NPI
- Copy of medical license
- Copy of driver's license or passport

Sign up for Structured Labs
Practice Fusion provides free integrations with numerous national and regional laboratories to streamline your practice's lab results and orders. Learn which laboratories we currently partner with by clicking here. One of the menu (not required) EHR incentive program criteria requires you to receive labs in structured format. Ask your Account Manager which lab partners can provide this functionality.
