  • Manually submit Dashboard and CQM report values to CMS
  • Core and Menu sections: Specify “This data was extracted only from patient records maintained using certified EHR technology”
  • CQM section: Make sure you do not choose to participate in the PQRS pilot

  • Review the CQM attestation instructions
  • When completing the core and menu section: specify that data was collected from patient records maintained using certified EHR technology
