Once you know you’re eligible to participate in Meaningful Use and you’ve signed up for your free EHR, you should register for the program on the CMS website. You have until the day you attest to complete registration. You can update your practice’s information any time in the CMS system. For more information, check the CMS registration guide.
You'll need the following information to register:
National Provider Identifier (NPI)
National Plan and Provider Enumeration System (NPPES) user ID and password
(the same information that’s used to log in to PECOS)
Practice Fusion’s 2014 Edition CMS Certification ID. Refer to Practice Fusion's Meaningful Use Dashboard for your selected reporting period's Certification ID.
CMS Tax Identification Number (TIN) to select where the payment will go

There are several ways to set up your Practice Fusion account before you start the program:
Add all practice members as users within your account with separate login credentials so everyone can help achieve Meaningful Use criteria. As long as you’re the provider who has signed for the encounter, you’ll still get credit in your Meaningful Use Dashboard.
Sending prescriptions electronically can help you meet several Meaningful Use measures at once.
Meaningful Use is based on all patients you see—not just those who are eligible for Medicare or Medicaid, so you should chart all patients in Practice Fusion, regardless of their insurance status. And remember to sign your chart notes for all patient visits in order to get credit in the Meaningful use Dashboard.
